I'm removing old undercoating from inside the finders and I noticed this
Looks like a washer but is it factory or dose it look like work has been done there

I saw it. Looks like a cheap way to plug a rust hole. Is that seam sealer globbed around it?

Looks like it was dropped/left at the factory before undercoating.

I've often found that the rust that you see if often just the beginning!

That is a bad place for rust, too. Thick steel with no easy way to keep water out. Maybe the windshield leaked or the A pillar had some rust and allowed water to drain down there?

A factory worker or a garage could have been installing a shock/part and set a washer on the fresh air vent.

Doesn't it look like a washer that goes on the screw used to attach the rear fender seal in that location.
