Title: Door switch ground when door open

When you open the door on a 2nd gen is the door switch grounded or is it fed 12 volts?

It's a ground.

takes ground from chassis through the threads, close circuit when open, then send to whatever signal needing open door signal ( dome light, ign key courtesy light flasher, map light etc... )
