Piston skirt failures

Curious how many long time engine builders have seen skirt failures when installing offset pistons the wrong way.  I'm told it's a big problem but it was once a time honored way to reduce piston friction.  I have done it in the past and it made it a little noisier maybe from piston slap but I never had a skirt come apart.  Was I just lucky?

My experience is the same as yours, I always turn them around (BB and SB) and never had a problem.
